About the Role

The position of Client Care Coordinator is a vital role in providing remote support to clients across various stages of treatment. This involves coordinating with clients, families, and service providers to ensure efficient delivery and documentation of wraparound services.

This role requires strong written and verbal communication skills, proficiency in Google Workspace or Microsoft Office, and the ability to work from a secure remote location. The ideal candidate will have experience in behavioral health, social services, or case management and be able to obtain a Level One Fingerprint Clearance Card.

Key Responsibilities
  • Serve as the primary point of contact for assigned clients and their families to support non-clinical needs and enhance treatment outcomes.
  • Coordinate all therapy and psychiatric appointments, ensuring accurate calendar management across clinical schedules.
  • Document all appointments and track no-shows, cancelations, and reschedules.
  • Maintain ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.
Requirements
  • Experience in behavioral health, social services, or case management (2+ years)
  • Strong written and verbal communication skills
  • Proficiency in Google Workspace or Microsoft Office
  • Reliable high-speed internet and ability to work from a secure remote location
  • Ability to obtain a Level One Fingerprint Clearance Card
