Job Overview

We are seeking a skilled professional to oversee the intake coordination process for new patients in a fast-paced home healthcare environment.

  • Key Responsibilities:
  • Coordinate the intake process, ensuring compliance with regulations and maintaining accurate documentation.
  • Collaborate with physicians and nurses to review and edit clinical notes, as well as manage referrals, authorization statuses, and admission workflows.
  • Serve as the primary liaison between field clinicians, doctors, and administrative teams to streamline documentation and facilitate timely patient care.
  • Utilize data management systems, such as DataSoft Logic and Axxess, to track and analyze documentation, compliance, and reporting workflows.

The ideal candidate will possess exceptional communication skills, both written and spoken, and have experience working in a similar role within the US healthcare sector. Additionally, they should be proficient in utilizing data management systems and familiar with industry protocols and standards.

Requirements
  • Clinical Experience: 3-5 years of Intake Coordination experience in the United States, preferably within California's home health sector.
  • Clinical Credentials: Registered Nurse (RN), Doctor (MD), or high-level clinical healthcare expert.
  • Administrative Experience: 1-2 years of experience in healthcare administration - ideally in home health.
  • Communication Skills: Flawless English communication skills, both written and spoken.
  • Data Management: Proven hands-on experience with DataSoft Logic and Axxess platforms.
  • Familiarity with Industry Protocols: Familiarity with Medicare/Medi-Cal intake protocols, Plan of Care documentation, and OASIS standards.
