The Remote Clinical Assistant role offers an opportunity to work in a dynamic and supportive environment, providing direct support to the Director of Nursing in overseeing clinical operations for home care services.

This position is responsible for assisting with documentation, compliance, scheduling, and care coordination to ensure the program runs efficiently and in alignment with regulatory standards.

The successful candidate will have a strong understanding of home health care processes, documentation, and patient care workflows.

Clinical Responsibilities

  • Review and maintain accurate patient care plans, medication administration records, and physician orders.
  • Follow up with physicians to ensure timely transcription into patient records.
  • Support the Director in tracking missed or overdue nursing visits and coordinate completion.
  • Assist with client admissions and discharges by preparing and verifying required documentation.
  • Generate compliance reports and assist with audit readiness.

Care Coordination

  • Monitor scheduling workflows to align PDN visits with care plans.
  • Serve as a liaison between nurses, patients, families, and providers to streamline communication.
  • Track staffing assignments and availability to support resource allocation.
  • Ensure staff credentialing (licenses, CPR, TB, etc.) are up to date.

Quality Assurance & Compliance

  • Support quality improvement initiatives by compiling reports and identifying documentation gaps.
  • Assist in reviewing clinical documentation for accuracy and completeness.
  • Provide feedback on documentation corrections or updates.

Qualifications: Education & Licensure

  • Current LPN license or higher (RN preferred).
  • Associate's or Bachelor's degree in Nursing or related healthcare field strongly preferred.
  • 2+ years of clinical experience in home health, PDN, or related nursing environment.
  • Prior experience in administrative or clinical support roles preferred.
