Healthcare Documentation Specialist Needed

Job Description

We are seeking a skilled and detail-oriented individual to join our team as a Healthcare Documentation Specialist. This vital role requires strong communication skills, attention to detail, and the ability to work independently in a focused environment.

Key Responsibilities:
  • Accurately document patient encounters directly into the Electronic Health Record (EHR) during or immediately after visits.
  • Capture relevant history, objective findings, assessments, treatment plans, and outcomes in clear, structured notes.
  • Use clinic templates and standards to ensure notes are billable, compliant, and ready for provider sign-off.
  • Prepare and edit patient notes, visit summaries, progress reports, and letters to referring physicians.
  • Maintain consistency of records across encounters and ensure timely chart updates.
  • Organize and track outstanding documentation for provider review.
  • Assist with documentation needed for medical necessity, insurance authorization, and claims support.
  • Enter or verify coding-related details (ICD/CPT as directed) and resolve documentation gaps that impact billing.
  • Coordinate with administrative staff to ensure clean claims and reduce rework.
Requirements

To be successful in this role, you must have:

  • Proven experience as a medical scribe, medical virtual assistant, or healthcare documentation specialist.
  • Strong written and spoken English communication skills.
  • Typing speed of at least 45 WPM with high accuracy.
  • Working knowledge of basic medical terminology (healthcare/clinical context).
  • Proficiency with EHR documentation and Microsoft Office (Word, Excel, Outlook), with strict adherence to HIPAA.
  • Ability to work independently in a focused, distraction-free environment.
Work Setup

This is a remote position that requires reliable internet and a quiet workspace. The working hours are Monday–Friday, 11:00 a.m.–8:00 p.m. Eastern Time, with a compensation rate of $7 per hour.
