Medical Documentation Specialist

This role aims to alleviate clinicians' documentation burden, allowing them to focus on hands-on patient care. The Medical Scribe will provide real-time scribing and accurate Electronic Health Record entries, ensuring records are complete, compliant, and billing-ready.

Key Responsibilities
  • Accurately document patient encounters directly into the Electronic Health Record during or immediately after visits
  • Capture relevant history, objective findings, assessments, treatment plans, and outcomes in clear, structured notes
  • Utilize clinic templates and standards to ensure notes are billable, compliant, and ready for provider sign-off
  • Prepare and edit patient notes, visit summaries, progress reports, and letters to referring physicians
  • Maintain consistency of records across encounters and ensure timely chart updates
  • Assist with documentation needed for medical necessity, insurance authorization, and claims support
  • Enter or verify coding-related details (ICD/CPT as directed) and resolve documentation gaps that impact billing
Requirements
  • Proven experience as a medical scribe, medical virtual assistant, or healthcare documentation specialist
  • Strong written and spoken English communication skills
  • Typing speed of at least 45 WPM with high accuracy
  • Working knowledge of basic medical terminology (healthcare/clinical context)
  • Proficiency with Electronic Health Record documentation and Microsoft Office (Word, Excel, Outlook), with strict adherence to HIPAA
