The OpportunityThis role requires proactive communication, exceptional organizational skills, and ability to manage complex patient care needs while maintaining confidentiality.
This position is remote and offers flexibility of working from home.
Key Responsibilities- Patient Care Coordination:
- Serve as liaison between patients and caregivers, ensuring seamless care.
- Monitor and maintain care quality by completing measures and quality assurance activities.
- Keep informed of patient care issues, including complaints, incidents, non-compliance, and changes in patient status.
- Handle escalated inquiries from patients and caregivers promptly and effectively.
- Ensure schedules reflect vendor authorization, patient preferences, and aide locations to maintain uninterrupted service.
- Communication & Documentation:
- Communicate with patients, families, physicians, and care teams regarding scheduling, appointment changes, and plan-of-care updates.
- Maintain accurate documentation for all caregivers and patients.
- Handle incoming calls, emails, and faxes regarding patient status, reporting, or other inquiries.
- Log and maintain proper documentation of events, patient progress, and care-related activities in accordance with departmental procedures.
- Maintain patient confidentiality, privacy, and dignity in all communications and actions.
- Collaboration & Support:
- Work closely with patient care coordinator on scheduling, availability, attendance, and operational issues.
- Manage phone calls and correspondence in a supportive, professional, and reassuring manner for patients, families, and healthcare providers.
- Provide additional support and responsibilities as assigned to meet organizational needs.
- Registered Nurse license.
- Must be bilingual in Spanish.
- Minimum of 3 years of case management or related experience.
- Excellent verbal and written communication skills.
- Strong interpersonal and customer service skills with empathy and patience.
- Ability to handle difficult situations professionally.
- Ability to function effectively in a fast-paced environment.
- Knowledge of exchange systems is highly preferred.
Remote working environment with international collaboration.
Essential RequirementsYou will need a reliable computer, noise-canceling headset, second monitor for productivity, and stable internet connection with backup internet connection for uninterrupted work.
