We are seeking a highly skilled and organized Care Coordinator to join our team. As a key member of our care coordination efforts, you will be responsible for ensuring patient records and medical documentation are managed with precision and empathy.
As a Care Coordinator, you will act as a liaison between families, healthcare providers, and hospitals, facilitating timely communication and document retrieval to support seamless care delivery. This role demands a sharp eye for detail, strong organizational skills, and a deep commitment to confidentiality, compliance, and compassionate service.
Key Responsibilities:- Manage patient documents and ensure timely, accurate distribution of medical records.
- Serve as the primary contact for communication with healthcare providers, hospitals, and medical facilities.
- Request and follow up on medical records, diagnostic reports, and imaging studies via calls and emails.
- Support families in scheduling visits and coordinating care delivery processes.
- Build meaningful relationships with parents as part of the support team.
- Collaborate with internal teams and departments to ensure a seamless experience.
- Bachelor's degree in nursing, healthcare administration, medical records management, or related field (preferred).
- Minimum 2+ year experience in administrative or operations roles (e.g., Operations Specialist, Care Coordinator).
- Strong organizational skills and attention to detail.
- Ability to multitask and solve problems creatively.

- Comfortable with dynamic responsibilities and cross-functional support.
- Proficient in CRM platforms such as Salesforce, with hands-on experience navigating Google Drive and other cloud-based tools for document management and communication.
