
Nurse Case Manager/Healthcare Specialist

The Medical File Reviewer/PHRN will prepare case files and Disability Benefits Questionnaires (DBQs) assigned in the client portal.
They will review and bookmark medical records, creating a synopsis to answer medical opinion questions based on the veteran's medical history.
- Assist medical providers during scheduled appointments by accessing, navigating, and preparing different apps in the exam room prior to the appointment.
- Remotely interview veterans via video or telephone conference platform.
- Transcribe medical history based on the veteran's answers to DBQ questions.
- Document physical examination findings in the appropriate section of DBQs.
- Complete all sections of DBQ spreadsheets after each appointment.
- Meet with medical providers to discuss assigned cases and determine final diagnosis per DBQ.
- Transfer data from DBQ spreadsheet to client portal.
- Submit DBQs in the client portal.
- Answer queries/addendum reports from clients' QAs.
- Coordinate with medical providers and team members regarding scheduled appointments.
Key Responsibilities include:

- Bachelor of Science in Nursing (BSN), active PHRN license.
- Good communication skills to assist veterans, medical providers, and colleagues.
- Able to work independently with minimal supervision.
- Flexible in work schedule changes as needed.
- Takes accountability for all assigned cases.
- Proficient in MS applications such as Word, Excel, and PowerPoint.
- Assists Lead and/or Trainer in improving process.
- Creates process-related templates as needed.
- Perform other job-related duties as assigned.
- Fiber connection with at least 20mbps download and upload speed.
- Available backup internet with minimum 15mbps download and upload speed.
- Main desktop/laptop must be i5 to i7 w/ 8GB RAM; 4th generation and above.
- Backup desktop/laptop must be i3 to i5 w/ 8GB RAM; 4th generation and above.
