Nurse Advocate

We are seeking a skilled Nursing Care Professional to connect with patients enrolled in our Chronic Care Management program. This is a vital role where you will empower patients with chronic conditions to live healthier, fuller lives.

You will make monthly outreach calls to patients, collaborate with them and their providers to design individualized care strategies, monitor symptoms, and escalate care when necessary. You will also work seamlessly with primary care providers, specialists, and community resources to ensure comprehensive care.

Key responsibilities include:

• Connecting with patients to understand their needs and develop personalized care plans.
• Collaborating with healthcare teams to ensure continuity of care.
• Maintaining accurate and timely patient records.
• Fostering strong relationships with patients and their families.

The ideal candidate has a valid RN license, at least 1 year of clinical nursing experience, and a strong understanding of chronic conditions such as diabetes, COPD, hypertension, and heart failure. Excellent communication, critical thinking, and organizational skills are essential for this role.

Bonus points if you have experience with Medicare Chronic Care Management documentation, motivational interviewing, or other patient engagement methods. We are looking for someone who can thrive in a collaborative team setting while working independently.
