Job Title: Patient Care Liaison

 

Job Description:

As a Patient Care Liaison, you will serve as the primary point of contact between patients and caregivers. Your role will involve ensuring seamless and continuous care by coordinating with patients, families, physicians, and healthcare teams.

Key responsibilities include patient care coordination, effective communication and documentation, and collaboration with the patient care coordinator to manage scheduling, availability, attendance, and operational issues.

Additionally, you will be responsible for maintaining accurate and up-to-date documentation for all caregivers and patients, handling incoming calls, emails, and faxes, and logging events, patient progress, and care-related activities in accordance with departmental procedures.

Maintaining patient confidentiality, privacy, and dignity is essential in all communications and actions.

Required Skills and Qualifications:
  • Qualifications:
    • Registered Nurse with NY License - USRN NY
    • Must be bilingual in Spanish
    • Minimum of 3 years of case management or related experience
    • Excellent verbal and written communication skills
    • Strong interpersonal and customer service skills with empathy and patience, particularly in working with the elderly population
    • Ability to handle difficult or sensitive situations professionally, both in person and over the phone
    • Ability to function effectively in a fast-paced and at times high-pressure environment
    • Knowledge of HHA Exchange is highly preferred
  • Education:
  • NY State Registered Nurse License
