Job Title: Healthcare Liaison

The Opportunity:

We are seeking a skilled healthcare liaison to serve as the primary point of contact between patients and caregivers.

This role requires strong communication, organizational, and problem-solving skills to ensure seamless care delivery.

Key Responsibilities:

Responsibilities may include additional tasks as required by departmental and organizational needs.

  • Patient Care Coordination:
    • Serve as primary liaison between aides and patients.
    • Monitor and maintain care quality.
    • Keep informed of patient care issues.
    • Handle escalated inquiries from patients and caregivers.
    • Ensure schedules reflect vendor authorization, patient preferences, and aide locations.
  • Communication & Documentation:
    • Communicate effectively with patients, families, physicians, and care teams.
    • Maintain accurate documentation for all caregivers and patients.
    • Handle incoming calls, emails, and faxes regarding patient status and reporting.
    • Log and maintain proper documentation of events and patient progress.
    • Maintain patient confidentiality and dignity.
  • Collaboration & Support:
    • Work closely with patient care coordinator on scheduling and operational issues.
    • Manage phone calls and correspondence in a supportive manner.
    • Provide support and responsibilities as assigned to meet organizational needs.
Requirements:
  • Registered Nurse with NY License.
  • Bilingual in Spanish.
  • Minimum of 3 years of case management or related experience.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and customer service skills.
  • Ability to handle difficult situations professionally.
  • Ability to function effectively in a fast-paced environment.
  • Knowledge of HHA Exchange is highly preferred.
