Job Title: Home Health Support Specialist

 

The Home Health Support Specialist will play a vital role in ensuring the smooth operation of our home care services. This position involves providing administrative and clinical support to our team, ensuring accurate documentation, compliance with regulations, and effective communication between patients, families, and healthcare providers.

 

Key Responsibilities:

  • Clinical & Administrative Support

    • Review and maintain patient care plans, medication administration records (MARs), and physician orders to ensure accuracy and compliance
    • Follow up with physicians for signed orders and ensure proper transcription into patient records
    • Support the Director of Nursing in tracking missed or overdue nursing visits and coordinate completion
    • Assist with client admissions and discharges by preparing and verifying required documentation
    • Generate and maintain compliance reports and assist with audit readiness

  • Quality Assurance & Compliance

    • Support quality improvement initiatives by compiling reports and identifying documentation gaps
    • Assist in reviewing clinical documentation for accuracy, completeness, and compliance with state/federal regulations

 

Required Skills and Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license or higher (RN preferred)
  • Associate's or Bachelor's degree in Nursing or related healthcare field strongly preferred
  • 2+ years of clinical experience in home health, PDN, or related nursing environment
  • Prior experience in administrative or clinical support roles preferred
