Key Responsibilities- Patient Care Coordination:
- Ensure a professional patient experience by serving as the first point of contact.
- Collect and update patient information accurately while maintaining confidentiality.
- Schedule appointments efficiently and conduct initial screenings.
- Manage patient records in electronic medical record systems.
- Respond to patient inquiries via phone, email, or chat.
- Provide pre-appointment instructions and ensure patients understand documentation requirements.
- Provider Outreach:
- Conduct provider outreach to present the company's value proposition and acquire partnerships.
- Maintain accurate outreach records in customer relationship management tools.
- Track and report key performance indicators (calls, emails, meetings, conversion rates).
- 1+ years of experience in care coordination, particularly in physical healthcare.
- Strong cold-calling and email outreach skills.
- Proficiency with customer relationship management tools and electronic medical record systems.
- Excellent verbal and written communication skills.
