About the Role

This position involves guiding healthcare providers to expand the company's presence. Key responsibilities include coordinating patient care, collaborating with providers, and managing outreach efforts.

Key Responsibilities
  • Patient Care Coordination:
    • Ensure a professional patient experience by serving as the first point of contact.
    • Collect and update patient information accurately while maintaining confidentiality.
    • Schedule appointments efficiently and conduct initial screenings.
    • Manage patient records in electronic medical record systems.
    • Respond to patient inquiries via phone, email, or chat.
    • Provide pre-appointment instructions and ensure patients understand documentation requirements.
  • Provider Outreach:
    • Conduct provider outreach to present the company's value proposition and acquire partnerships.
    • Maintain accurate outreach records in customer relationship management tools.
    • Track and report key performance indicators (calls, emails, meetings, conversion rates).
Requirements
  1. 1+ years of experience in care coordination, particularly in physical healthcare.
  2. Strong cold-calling and email outreach skills.
  3. Proficiency with customer relationship management tools and electronic medical record systems.
  4. Excellent verbal and written communication skills.
