Job Descriptions

  • Conduct comprehensive psychosocial assessments for patients and families to identify emotional, financial, and social needs.
  • Develop and implement individualized care plans, collaborating with medical teams, nurses, and other healthcare professionals.
  • Provide counseling and crisis intervention to patients and families coping with illness, hospitalization, and treatment.
  • Assist patients in navigating financial assistance programs, health insurance processes, and government aid (e.g., PCSO, DSWD).
  • Coordinate referrals to community resources such as support groups, rehabilitation services, or long-term care facilities.
  • Advocate for patient rights and ensure culturally sensitive care.
  • Maintain accurate case records and documentation in compliance with hospital policies and regulatory standards.
  • Participate in multidisciplinary team meetings to contribute social work expertise to patient care planning.
  • Support hospital health promotion programs and outreach initiatives when needed

Qualifications:

  • Must be a Registered Social Worker (RSW) with a valid PRC license.
  • Bachelor's Degree in Social Work.
  • At least 1-2 years of experience in a healthcare or hospital setting preferred.
  • Strong counseling, case management, and crisis intervention skills.
  • Knowledge of hospital social services programs, community resources, and government aid processes.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team.
