Job Summary

Social Work Assistant is assisting the head, social work in planning and implementing programs to meet the social and emotional needs of patients and patients' families in a health care setting. He/She helps the head, social work develops plans for patient care after release from enrollment of the patient until the postpartum care period. Being a Social Work Assistant provides social services to patients/clients and their families. Collaborates with physicians, nurses, midwives, patients, and their families to assess patient social needs and monitor progress for medical treatment and any necessary holistic interventions such as feeding programs, mental health support groups, financial aid, partnership with local NGOs and the like. He/ She coordinates planning for the holistic component of postpartum care.

Key Responsibilities:
  1. Setup and maintain all aspects of the Social Work Program including

■ Case Reports

■ Program Reports

■ Budget allocation
2. Establish strong case reports through home visits, hospital visits, and other patient-assistive activities outside of Glory Reborn.
3. Establish education modules for the selected patient curriculum
4. Facilitate Glory Reborn orientation for programs and services for first-time patients during initial interviews.
5. Establish a partnership with other non-profit organizations, government, and private entities to increase the services that Glory Reborn provides
6. Continue to coordinate with local NGO's to assist their clients with the services of Glory Reborn Org. and to continue to be active partners together for our clients.
7. Facilitate and implement patient visits for patients transported to the hospital
8. Actively participate in monthly meetings, outreaches, and other required clinical activities with a positive attitude.
9. Assist the SW Head with the SW monthly accomplishment reports.
10. Submit to the SW Manager the monthly patient's story (1-2 stories).
11. Making sure that the patient's chart is complete on SW forms.
12. Assist patients in difficult or emergency situations, including processing of paperwork and other activities for situations like:

■ Request for blood

■ Death of Infant

■ Hospitalization of Infant

■ And other emergency situations of the patient's needs

  1. Assists in the regular update of the DSWD registration and licensing, and accreditation of the organization.
  2. Assists in the signing off of the medical charts of all discharged patients to ensure its completeness prior to storing in the Medical Charts cabinet.
  3. Renders TeleHealth care via phone, Zoom, and social media platforms to clients in an effective and efficient manner and in accordance with government regulations
  4. Perform other functions and responsibilities, which may be assigned by the immediate superior or the CEO.
  5. Collaboratively responds to and/or acknowledges all emails sent out by the Management or Department. Keeps up to date on the department & organizational memorandum for policies, changes, programs, and events through timely response & organizational emails and other communications.
Qualifications:
  1. A Licensed Social Worker
  2. Maintain a high level of confidentiality
  3. Communicate clearly and concisely in both written and oral form
  4. Perform clerical and social work tasks quickly and accurately
  5. Coordinate daily activities and/or schedule
  6. Work independently and organize complex clerical tasks
  7. Ability to be flexible and adaptable in a variety of situations
  8. Remain calm under trying circumstances and work with frequent interruptions
  9. Maintain efficient office procedures and a system for keeping track of requested actions and reports
  10. Meet strict timelines and perform multiple tasks
  11. Work harmoniously with individuals and groups of employees
  12. Skill and knowledge in the operation, use, and care of office equipment
  13. Knowledge and proficiency in the use of technology (i.e., computers, word processing, database, spreadsheet programs, and PowerPoint)
  14. Promote proactive and effective communication among workmates and patients.
  15. Accomplish work responsibilities with minimum supervision
  16. Knowledge of the functions of the various parts of the assigned department
  17. Has an understanding of ministry and biblical principles and is willing to work in a Christian non-profit organization to invest in the individual lives of our clients.
