dialysis nurse

1. Receives patients for dialysis and lists all the pertinent data to the patients personal information logbook

2. Provides direct care to patients undergoing hemodialysis based on the rules and regulations established by the Department of Health and approved company policies and procedures.

3. Checks and prepares all materials needed and ensures its safety and availability before the start of every dialysis.

4. Reads and carries out doctors' orders and ensures that dialysis is performed accordingly.

5. Does proper nursing assessment before, during and after every dialysis session.

6. Takes vital signs and weighs the clients accurately

7. Assesses, inspects, regulates and monitors all machine settings and parameters. Computes the UF goal and inputs it correctly inputs, regulates, and monitors all the machine settings and parameters in accordance to the patients' present health condition and nephrologists' orders.

8. Prepare patient's access using the required standards for arterio-venous fistula and grafts and also external access preparation.

9. Communicates with the Doctor regarding identified client – related problems and initiates interventions for the same.

10. Completes the complication log book every seen client – related problem during and after dialysis with their corresponding nursing actions and clients' response to care\

11. Administers medications as ordered and properly documents the same observing the 10R's of giving right medication.

12. Updates the patients' medication record every dialysis session

13. Performs, monitors and records pre-, intra-, and post dialysis treatment vital signs;

14. Initiates/Performs Cardio-Pulmonary Resusitation (CPR) in the event of cardiac and/or pulmonary arrest and sees to it that all activities are supervised, organized and documented.

15. Updates patients' records during and after every dialysis session.

16. Inventories E-cart, E-kit medications and unit stocks/supplies at the end of every shift

17. Protects patients from accidents, hazards and potential infections.

18. Observes the line of communication and applies appropriate communication and techniques in reporting, recording, and relaying information or data.

19. Provides accurate information and proper health education to the patients and significant others regarding the plan of care.

20. Observe proper attitude and decorum during duty.

21. Does proper endorsement of clients and relays completely the nursing actions done.

GENERAL QUALIFICATIONS STANDARD:

1. Education/ Professional / Qualification:

· A duly license Registered nurse by the Professional Regulatory Commission

· Certified by Renal Nurses of the Philippines

· Has certificate of training in the nursing care of Renal Hemodialysis clients from competent training providers

· Undergone updated training in ACLS and BLS conducted by competent authorities or professional organizations

· Completed and certified IV therapy nurse

· Attends update related to nursing staff's duty

· Physically fit, no health-related problems

· Negative Hepa B and C screening results, with at least 1000 anti-hbs with titer

· Computer literate

· Hardworking with good moral conduct
