URGENT Clinical Support Officer

The Clinical Support Officer will play a key role in working under the guidance of the National Transitions and Clinical Manager to provide clinical support. Working closely with our house leaders, internal and external stakeholders, the role will contribute to ensuring holistic care is provided to participants through a person-centred approach.

Position Accountabilities

  • Assist with reviewing and developing participant care plans
  • Assist with reviewing and developing participant risk profiles
  • Renew Comprehensive Health Assessment Program (CHAPs) tools
  • Assist team leaders with medical or general practitioner appointments, attending where possible
  • Review medical appointments sheets and feedback to clinical team on medical recommendations by doctor or allied help profiessional
  • Liaise with external specialist via telehealth to gather and put in place key care initiatives for our participants
  • Assist with RPA monitoring, reporting and panel hearings
  • Assist in conducting risk assessments and reportable incidents
  • Partake in on-call duties where required

Education, Experience, Skills and Qualifications

  • MUST HAVE experience with Australian clients/settings
  • Tech savy, knows how to use MS Office and other CRM tools
  • Experience with working within a clinical support officer role
  • Bachelor of Science in Nursing
  • At least 2 years experience in a clinical setting

  • Excellent communication skills in being able to build rapport with a varietyof internal and external stakeholders.
  • Understanding of NDIS (Disability Industry) standards and applications with aspects such as reporting, medications, care plans and restrictive practices.
  • Self-motivated, can work independently and organised in ability to prioritise and plan effectively.
  • A person-centric mindset.
