Medical Records Associate

Duties and Responsibilities:

  • Receive, review, and process medical records, including admission, discharge, and transfer documentation.
  • Organize and file medical records both in physical and electronic formats accurately.
  • Maintain patient records in accordance with hospital policies, legal standards, and confidentiality regulations.
  • Retrieve medical records promptly upon request by authorized personnel.
  • Ensure completeness and accuracy of patient records, including diagnoses, treatments, and physician notes.
  • Encode patient information and updates into hospital electronic health record (EHR) systems.
  • Assist in auditing medical records for compliance, accuracy, and completeness.
  • Support health information reporting and statistical data collection.
  • Handle sensitive patient information with utmost confidentiality and professionalism.
  • Coordinate with clinical staff, billing, and administration to ensure smooth record-keeping processes.
  • Participate in the preparation of medical records for external requests, including insurance claims or legal inquiries.

Qualifications:

  • At least a high school diploma; an associate's degree in Health Information Management, Medical Records, or related field is preferred.
  • Basic knowledge of medical terminology and hospital procedures.
  • Computer literate, with familiarity in using electronic health record (EHR) systems and MS Office applications.
  • Attention to detail and high level of accuracy.
  • Strong organizational and filing skills.
  • Good communication and interpersonal skills.
  • Ability to maintain confidentiality and work with sensitive information.
  • With ICD10 Is plus

Working Conditions:
