We are seeking a dedicated and detail-oriented Home Health Clinical Documentation Auditor to join our team.
This position is critical in ensuring the accuracy and compliance of clinical documentation within our home health services.
The ideal candidate will have extensive experience with OASIS and a strong understanding of home health regulations.

Responsibilities:
- Conduct thorough audits of clinical documentation to ensure compliance with state and federal regulations, including OASIS assessments and POC Information
- Provide live transcription services during calls with clinicians, accurately capturing details for documentation purposes
- Professionally handle inbound and outbound calls for clinicians, providing guidance, support, and clarification on documentation requirements and processes
- Duties & Responsibilities:
- Obtain and follow up on Face-to-Face (F2F) encounter documentation from physicians
- Ensure timeliness, accuracy, and completeness of all home health required documents
- Review and monitor patient charts for eligibility, physician orders, recertifications, and plan of care compliance
- Coordinate with physicians, clinicians, and internal departments for missing or pending requirements
- Track and escalate any delays, missing signatures, expiring orders, or compliance risks
- Collaborate with intake/admissions team to support case initiation readiness
- Maintain organized documentation logs and update EMR accurately
- Exercise critical thinking to prevent denials, delays, or compliance issues
- Ensure all communication and follow-ups are professional, proactive, and documented
Requirement:
- Minimum of 1 year of experience with home health clinical documentation, specifically with OASIS
- Strong communication skills, both verbal and written. Ability to handle live transcription and provide real-time documentation support
- In-depth understanding of home health regulations, including OASIS, face-to-face requirements, NOMNC, and order requirements
- Proficiency with electronic health records (EHR) systems and other relevant documentation tools
