Medical Documentation Specialist - Home Health

This is a specialized role that requires experience in medical documentation. The successful candidate will work closely with nurses and other healthcare professionals to assist with pre-charting and post-charting of comprehensive assessments in MatrixCare EMR.

Key responsibilities include:

  • Completing pre-charting on comprehensive home health assessments in MatrixCare EMR
  • Assisting with post-charting by accurately documenting clinical notes and required details
  • Ensuring that documentation is complete, accurate, and compliant with regulatory standards
  • Working collaboratively with nurses to support the review and finalization of patient records
  • Maintaining strict confidentiality of patient health information in accordance with HIPAA

The ideal candidate will have hands-on experience with home health workflows, strong EMR skills, and excellent attention to detail.

Required Skills and Qualifications

To be considered for this role, you should have:

  • Education: Associate's degree or higher in a healthcare-related field
  • Experience:
    • Minimum 1–2 years of experience as a Medical Scribe or similar role
    • Prior experience in home health documentation required
    • Hands-on proficiency with MatrixCare EMR strongly preferred
  • Strong understanding of home health processes, including OASIS and care planning
  • Excellent written communication and data entry skills
  • High attention to detail and accuracy in medical documentation
