We are seeking a highly skilled Medical Documenter to support our clinical team in home health settings.

The ideal candidate will have hands-on experience with home health workflows, strong EMR skills, and excellent attention to detail.

The successful candidate will work closely with nurses to assist with documentation, focusing on pre-charting and post-charting of comprehensive assessments in MatrixCare EMR. All entries will be reviewed, signed, and finalized by a licensed nurse.

Key Responsibilities:

  • Complete pre-charting on comprehensive home health assessments in MatrixCare EMR.
  • Assist with post-charting by accurately documenting clinical notes and required details.
  • Ensure that documentation is complete, accurate, and compliant with regulatory standards.
  • Maintain strict confidentiality of patient health information in accordance with HIPAA.
  • Keep up-to-date with home health documentation requirements and best practices.
Required Skills and Qualifications

This role requires a strong understanding of home health processes, including OASIS and care planning.

  • Education: Associate's degree or higher in healthcare-related field preferred.
  • Experience:
    • Minimum 1–2 years of experience as a Medical Scribe or similar role.
    • Prior experience in home health documentation required.
    • Hands-on proficiency with MatrixCare EMR strongly preferred.
  • Excellent written communication and data entry skills.
  • High attention to detail and accuracy in medical documentation.
  • Ability to work independently in a remote, fast-paced environment.
  • Strong organizational and multitasking skills.
