
Family Care Coordinator

We are seeking a highly skilled professional to join our team as a Family Care Coordinator. This role will be responsible for empowering underserved families to access care by collecting, reviewing, and tracking required documentation.
As a Family Care Coordinator, you will work with real families who face real human problems. Your primary goal will be to guide these families through the documentation process, making outbound calls and responding to inbound inquiries in a timely and professional manner.
- Collect and review required documentation from caregivers and families (e.g., IDs, certifications, health records).
- Make outbound calls and respond to inbound inquiries to guide families through the documentation process.
- Handle high-volume phone interactions daily while maintaining professionalism, empathy, and efficiency.
- Ensure all documents are accurate, complete, and compliant with state and Medicaid/Medicare regulations.

- You have extensive experience in customer service, administrative, or healthcare support roles.
- You can handle high-volume inbound and outbound calls in a fast-paced environment.
- You possess excellent organizational skills and attention to detail when reviewing documents.
- You have clear and professional communication skills, both verbal and written.
- You are comfortable using CRM systems, shared drives, or document tracking tools.
- You can manage multiple tasks, prioritize follow-ups, and meet deadlines.
- You have a compassionate and patient approach when working with families and caregivers.
