The Outpatient Care Coordinator Specialist plays a pivotal role in delivering high-quality, patient-centered support across the entire treatment continuum. This includes intake, active care coordination, appointment scheduling, and discharge planning.

Key Responsibilities:
  • Serves as the primary point of contact for assigned outpatient clients and their families to address non-clinical needs and enhance treatment outcomes.
  • Cools and schedules all in-person and virtual therapy and psychiatric appointments, ensuring proactive communication with providers and clients regarding attendance.
  • Documents all appointments and maintains accurate calendar management across clinical schedules.
  • Tracks and reports no-shows, cancellations, and reschedules; notifies relevant parties of applicable billing charges.
  • Monitors client participation in scheduled services and follows up on attendance, engagement, and missed appointments.
  • Cools and discharges clients, ensuring timely completion of all required documentation.
  • Identifies barriers and needs related to housing, education, transportation, healthcare, legal services, and social determinants of health; links clients to relevant resources.
  • Maintains ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.
  • Participates in interdisciplinary team meetings to review and support treatment plan goals.
  • Escalates urgent or critical issues to the Clinical Director immediately.
  • Upholds professional ethics and confidentiality standards in all communications and records.
  • Utilizes electronic systems to track and report client-related activities.

To excel in this role, candidates should possess strong backgrounds in behavioral health, social services, or case management, along with excellent written and verbal communication skills. Proficiency in Google Workspace and/or Microsoft Office is essential, as well as reliable high-speed internet and the ability to work from a secure remote location.
