Healthcare Documentation Specialist

This role provides high-quality documentation support to a leading physical therapy practice in the U.S., focusing on patient-centered care and personalized service.

Key Responsibilities
  • EHR Documentation: Accurately document patient encounters directly into the EHR during or immediately after visits, capturing relevant history, objective findings, assessments, treatment plans, and outcomes in clear, structured notes.
  • Clinical Correspondence & Records: Prepare and edit patient notes, visit summaries, progress reports, and letters to referring physicians, maintaining consistency of records across encounters and ensuring timely chart updates.
  • Billing & Insurance Support: Assist with documentation needed for medical necessity, insurance authorization, and claims support, entering or verifying coding-related details (ICD/CPT as directed) and resolving documentation gaps that impact billing.
  • Data Quality, Timeliness & Compliance: Perform accuracy checks for patient identifiers, visit details, and plan of care before submission, adhering to HIPAA and clinic privacy/security protocols.
Requirements
  • Proven experience in healthcare documentation, clinical correspondence, and data quality control.
  • Strong written and spoken English communication skills.
  • Typing speed of at least 45 WPM with high accuracy.
  • Working knowledge of basic medical terminology (healthcare/clinical context).
  • Proficiency with EHR documentation and Microsoft Office (Word, Excel, Outlook), with strict adherence to HIPAA.
Work Setup & Hours
  • Remote position.
  • Must have a reliable internet connection and a quiet workspace.
  • Required to provide own computer with Intel Core i5 or equivalent or higher.
  • Hours: 40 hours per week, Monday–Friday, 11:00 a.m.–8:00 p.m. Eastern Time.
Compensation
  • $7 per hour.
  • No benefits package included.
