Care Coordinator - Registered Nurse

We are seeking a highly skilled Care Coordinator - Registered Nurse to join our team in improving the health outcomes of patients across the United States. This full-time night shift role is ideal for nurses who are highly organized, fluent in English, and committed to making a difference in patients' lives.

This challenging position plays a critical role in proactive support, education, and follow-up care to help patients avoid unnecessary hospitalizations through timely interventions and effective disease management.

  • Serve as the primary care coordinator for assigned patients in the US
  • Conduct telephonic outreach and/or in-person assessments based on program guidelines
  • Collaborate with physicians, specialists, and healthcare professionals to create individualized care plans
  • Educate patients and caregivers on disease management, medications, and lifestyle adherence
  • Track key health indicators and intervene early to prevent ER visits or hospital readmissions
  • Coordinate services across multiple healthcare providers, facilities, and support systems
  • Maintain timely, accurate records in electronic health records or program-specific platforms
  • Report clinical outcomes and escalate high-risk cases as needed
  • Participate in care team meetings and contribute to ongoing planning and improvements
Requirements
  • Active RN license
  • Experience in chronic care, patient education, or case management
  • Excellent English communication skills
  • Strong clinical judgment and attention to detail
  • Comfortable working independently and using telehealth systems
  • Willing and able to work night shift hours
  • Tech-savvy and familiar with electronic documentation
