As a dedicated professional, you will play a pivotal role in supporting the Private Duty Nursing (PDN) Director of Nursing in overseeing clinical operations for home care services.

This position is responsible for assisting with documentation, compliance, scheduling, and care coordination to ensure the PDN program runs efficiently and aligns with regulatory standards.

The ideal candidate must hold an LPN license (or higher) and possess a strong understanding of home health care processes, documentation, and patient care workflows.

Clinical & Administrative Support

  • Review and assist in maintaining patient care plans, MARs, and physician orders to ensure accuracy and compliance.
  • Follow up with physicians for signed orders and ensure proper transcription into patient records.
  • Support the PDN Director in tracking missed or overdue nursing visits and help coordinate completion.
  • Assist with client admissions and discharges by preparing and verifying required documentation.
  • Generate and maintain compliance reports and assist with audit readiness.

Care Coordination

  • Monitor and help manage scheduling workflows to ensure PDN visits are aligned with care plans.
  • Serve as a remote liaison between nurses, patients, families, and providers to streamline communication.
  • Track staffing assignments and availability to support the Director of Nursing in resource allocation.
  • Ensure staff credentialing (licenses, CPR, TB, etc.) are tracked and up to date.

Quality Assurance & Compliance

  • Support quality improvement initiatives by compiling reports and identifying documentation gaps.
  • Assist in reviewing clinical documentation for accuracy, completeness, and compliance with state/federal regulations.
  • Provide feedback to field nurses (under supervision of the PDN Director) on documentation corrections or updates.

Training & Education Support

  • Help facilitate remote orientation and training sessions for nursing staff.
  • Track completion of required in-services and continuing education.
  • Assist in distributing policy updates, clinical reminders, and compliance alerts to nursing staff.

Qualifications: Education & Licensure

  • Current and valid Licensed Practical Nurse (LPN) license or higher (RN preferred).
  • Associate's or Bachelor's degree in Nursing or related healthcare field strongly preferred.
  • 2+ years of clinical experience in home health, PDN, or related nursing environment.
  • Prior experience in administrative or clinical support roles preferred.
