
Home Healthcare Operations Specialist

We're seeking a seasoned Intake Coordinator to oversee the intake process for new patients in home healthcare operations. As a critical bridge between physicians, nurses, patients, and administrative staff, you'll ensure seamless coordination and compliance with regulatory requirements.
Key Responsibilities:Manage and coordinate the intake process to ensure timely and accurate patient care
Review and write high-level clinical notes in collaboration with physicians and nurses
Track and manage referrals, authorization statuses, and admission workflows daily
Support all administrative operations, including scheduling, documentation, and internal communications
Serve as the main liaison for field clinicians and doctors to streamline documentation and provide timely patient care
Utilize DataSoft Logic and Axxess systems to manage documentation, compliance, and reporting workflows
Communicate clearly and professionally with patients, families, clinicians, and administrative teams
The ideal candidate will possess 3–5 years of experience in Intake Coordination in the United States, preferably within California's home health sector. Clinical credentials such as Registered Nurse (RN), Doctor (MD), or high-level clinical healthcare expert are required. Additionally, 1–2 years of administrative experience in healthcare—ideally in home health—is preferred.
