About the Role

This is a Healthcare Navigation Specialist role that involves guiding and supporting healthcare providers to ensure seamless onboarding, engagement, and collaboration for improved patient outcomes.

Key Responsibilities
  • Care Coordination (80%)
    • Serve as the first point of contact for patients, ensuring a professional and welcoming experience.
    • Collect, verify, and update patient information accurately while maintaining confidentiality.
    • Schedule appointments efficiently and conduct initial screenings to direct patients appropriately.
    • Manage patient records in electronic medical record systems, adhering to HIPAA regulations.
    • Respond to patient inquiries via phone, email, or chat with empathy and clarity.
    • Provide pre-appointment instructions and ensure patients understand documentation and insurance requirements.
    • Coordinate with healthcare providers, insurance teams, and staff to ensure smooth patient flow.
  • Provider Partnership (20%)
    • Establish provider partnerships by presenting the company's value proposition and developing strong relationships.
    • Handle high volumes of outbound calls to engage with healthcare providers.
    • Maintain accurate outreach records in HubSpot CRM and track key performance indicators.
    • Share insights on prospect feedback and market trends to inform strategies.
