Job Description

The Outpatient Clinical Liaison plays a pivotal role in delivering comprehensive support services to clients throughout their treatment journey. This position requires a unique blend of clinical expertise, interpersonal skills, and technical proficiency.

Key Responsibilities:
  • Provide empathetic client-centered support across all stages of treatment, including intake, active care coordination, appointment scheduling, and discharge planning.
  • Collaborate closely with internal teams and external service providers to ensure efficient delivery and documentation of wraparound services.
  • Serve as the primary point of contact for assigned outpatient clients and their families, addressing non-clinical needs and enhancing treatment outcomes.
Required Skills and Qualifications:
  • Minimum 2 years of experience in behavioral health, social services, or case management.
  • Strong written and verbal communication skills, with ability to articulate complex information in a clear, concise manner.
  • Proficiency in Google Workspace and/or Microsoft Office, with ability to utilize electronic systems to track and report client-related activities.
