Clinical Support SpecialistJob Summary:
The Clinical Support Specialist provides direct support to the Director of Nursing in overseeing clinical operations for home care services. This role is responsible for assisting with documentation, compliance, scheduling, and care coordination to ensure the program runs efficiently and in alignment with regulatory standards.
Key Responsibilities:
- Support Services:
- Review and maintain patient care plans, medication administration records, and physician orders to ensure accuracy and compliance
- Follow up with physicians for signed orders and ensure proper transcription into patient records
- Support the Director in tracking missed or overdue nursing visits and coordinate completion
- Assist with client admissions and discharges by preparing and verifying required documentation
- Generate and maintain compliance reports and assist with audit readiness
- Care Coordination:
- Monitor and manage scheduling workflows to ensure field nurse visits align with care plans
- Serve as a remote liaison between nurses, patients, families, and providers to streamline communication
- Track staffing assignments and availability to support the Director in resource allocation
- Ensure staff credentialing (licenses, CPR, TB, etc.) are tracked and up to date
- Quality Assurance & Compliance:
- Support quality improvement initiatives by compiling reports and identifying documentation gaps
- Assist in reviewing clinical documentation for accuracy, completeness, and compliance with state/federal regulations
- Provide feedback to field nurses on documentation corrections or updates
- Training & Education Support:
- Facilitate remote orientation and training sessions for nursing staff
- Track completion of required in-services and continuing education
- Assist in distributing policy updates, clinical reminders, and compliance alerts to nursing staff
