
Outpatient Support Specialist

Job Overview
The Outpatient Case Manager is responsible for providing remote support to clients throughout their treatment journey. This role ensures the efficient delivery and documentation of wraparound services by coordinating with clients, families, internal teams, and external service providers.
Key Responsibilities:
- Serves as the primary point of contact for assigned outpatient clients and their families to support non-clinical needs and enhance treatment outcomes.
- Coordinates all in-person and virtual therapy and psychiatric appointments.
- Documents all appointments and ensures accurate calendar management across clinical schedules.
- Tracks and reports no-shows, cancelations, and reschedules; notifies finance of applicable no-show charges for billing.
- Maintains ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.
- Actively participates in interdisciplinary team meetings to review and support treatment plan goals.
Requirements
- 2+ years of experience in behavioral health, social services, or case management.
- Strong written and verbal communication skills.
- Proficiency in Google Workspace and/or Microsoft Office.
Preferred Qualifications
- Associate's or Bachelor's degree in Social Work, Psychology, or related field.
- Experience working with adolescents and families.
