Medical Records Specialist

Key Responsibilities:
• Compile medical statistics, such as admissions, discharges, deaths, and treatment types.
• Abstract information for medical records.
• Analyze records with a minimum of 95% accuracy.
• Perform quality checks on scanned medical records.
• Accurately complete daily, weekly, monthly, and quarterly reports.
• Verify patient identifiers on scanned pages.
• Ensure scanned documents belong to the same patient with the same date of service.
• Review electronic images for quality, order, and rotation.
• Conduct quality assurance and quality improvement on scanned/indexed documents.
• Enter, edit, and track medical record deficiencies.
• Communicate with physicians to resolve deficiencies.
• Review medical records edited by physicians and update the HIM system.
• Compile physician record completion statistics and notification letters.
• Produce and distribute delinquency notification/suspension letters.
• Monitor records submitted for reanalysis.
• Ensure completion of existing file deficiencies and verify accuracy of information submitted.
• Serve as a liaison to physicians regarding incomplete records and documentation.
• Facilitate chart availability and readiness for coding.
• Ensure patient's records have been received upon discharge.
• Facilitate the completion of birth certificates.
• Assist patients with patient portal questions and password re-sets.
• Assist visitors and physicians, and answer phone as needed.
• Monitor the transcription system and coordinate the ROI function.
• Ensure the integrity and quality of documents scanned into the document management system.
• Track unidentified documents, re-scan documents, and assign document types to unknown documents.
• Ensure all discharged records are received from units for document imaging.
• Perform follow-up of missing documents and verification that all charts have been received and scanned.
• Continuously meet all turnaround times as requested and per Departmental policy.
• Analyze patient's records for needed documentation by the physician.
• Responsible for the physician suspension process and record deficiency tracking.
• Operate computer to enter and retrieve data.
• Accept subpoenas after verifying their validity.
• Ensure all documentation is accurate and in the appropriate format to meet Joint Commission accreditation standards.
• Review the integrity of the Master Patient Index (MPI) to provide an accurate database.
• Combine patient records that are duplicated.
• Handle all incoming calls regarding registration issues due to duplicate or incorrect number assignment.
• Maintain log of activities.
• Meet or exceed productivity standards.
• Responsible for cross-training peers.
• Assist the HIM Department and various work units during times of staff shortage or high volumes.
• Responsible for other duties and special projects as assigned by management.
• Maintain strict physician and patient confidentiality.
• Follow all federal, state, industry, and hospital guidelines for release of information.
