
Remote Clinical Coordinator

As a Remote Clinical Assistant, you will be responsible for providing direct support to the Private Duty Nursing Director of Nursing in overseeing clinical operations for home care services. This role requires a strong understanding of home health care processes, documentation, and patient care workflows.
This position involves reviewing and maintaining patient care plans, MARs, and physician orders to ensure accuracy and compliance. You will also be required to follow up with physicians for signed orders and ensure proper transcription into patient records. Additionally, you will support the PDN Director in tracking missed or overdue nursing visits and help coordinate completion.
Clinical and administrative support is a key aspect of this role. You will assist with client admissions and discharges by preparing and verifying required documentation. Furthermore, you will generate and maintain compliance reports and assist with audit readiness.
You will serve as a remote liaison between nurses, patients, families, and providers to streamline communication. You will also track staffing assignments and availability to support the Director of Nursing in resource allocation. Moreover, you will ensure staff credentialing (licenses, CPR, TB, etc.) are tracked and up to date.
This role requires quality assurance and compliance expertise. You will support quality improvement initiatives by compiling reports and identifying documentation gaps. Additionally, you will assist in reviewing clinical documentation for accuracy, completeness, and compliance with state/federal regulations. You will provide feedback to field nurses on documentation corrections or updates.
You will also play a crucial role in training and education support. You will help facilitate remote orientation and training sessions for nursing staff. You will track completion of required in-services and continuing education. Furthermore, you will assist in distributing policy updates, clinical reminders, and compliance alerts to nursing staff.
To be successful in this role, you must have a current and valid Licensed Practical Nurse (LPN) license or higher. Prior experience in administrative or clinical support roles is preferred. Additionally, 2+ years of clinical experience in home health, PDN, or related nursing environment is required.
