In this role as a Patient Advocate Coordinator at Private Advertiser, you will be responsible for serving as the primary point of contact between our Care Management programs and members. Under general supervision, you will handle inbound and outbound calls with eligible members to provide information about program features and services.
Main Responsibilities
• Gather and record member information in accordance with policies and procedures via telephone.
• Encourage members to participate in Care Management programs by providing information about program features, outlining value, and explaining available services.
• Record call details and utilize call scripts as needed based on client requirements.
• Direct member inquiries to appropriate clinical staff.
• Initiate referrals and manage consults from clinicians for participants registered in Care Management.
• Gather information from hospitals, health plans, physicians, patients, vendors, and other referral sources.
• Contact physician offices to obtain demographic information or related data as needed.
• Mentor less experienced Health Program Advocates.
Requirements
The ideal candidate will have a high school diploma or equivalent and a minimum of 6 months experience; or any combination of education and experience which would provide an equivalent background.
