Patient Navigator

Job Descriptions
Validates/approves patient enrollment applications
Orients patients on program scheme and details, assists on inquiries/clarifications
Provides patient support with any inquiries or AE management counselling, if needed
Initiate contact with patients who missed their dose
Visits hospitals to collect enrollment forms (hardcopy) 2 days in a month
Coordinates with hospital pharmacists on sales transactions/medicine dispensing collection and clarifications
Supports hospital pharmacists on program or tools inquiries
Ensure compliance with relevant regulations, guidelines, and industry standards on patient support programs
Perform tasks assigned by the Patient Solutions Manager
Qualification:
Must be a Registered Nurse with Active license
At least 2 years’ experience in a hospital setting
Proficient in MS Office (excel)
Skills/Competencies required
Excellent communication and interpersonal skills to effectively collaborate with patients, families, and the healthcare team
Experience in handling onco medicine.
With medical thinking skills.
Must be keen to details and able to deliver effectively with minimal supervision and capable of multi-tasking.
Computer skills
Exhibits excellent time management.
