It takes a village to care for our patients, and as a Care Coordinator (CC), you will be at the heart of all the action. Our CCs work together as a team to facilitate the planning and organizing of all critical activities for our patients. The patients we care for are some of the most complex and need an excellent care team to help manage their chronic conditions. The Care Coordinator works closely with Nurse Practitioners and RN Case Managers to ensure our patients receive the highest quality care.

Location:

Commute to DaVita Clinics and partner practices in the Lincoln, NE metro areas. Mileage is reimbursed. MUST have reliable personal transportation.

Position Details:

Working in clinic 3-4 days per week, with remote work on remaining days.

Schedule:

Full-Time, Monday to Friday. Daily start time between 7:30 and 8:00 AM. Flexibility required.

Responsibilities:

• Complete non-clinical screenings and escalate as needed.
• Identify eligible resources for patients and navigate resources.
• Assist with referral processes and coordinate provider appointments.
• Support the clinical team in moving patients through the continuum of care by communication and task completion.
• Implement and document discharge care plans, including referral forms, clinical info gathering, chart initiation, and appointment scheduling.
• Communicate with patients regarding scheduling requirements.
• Maintain accurate, complete, and confidential documentation.
• Ensure timely delivery of documents while respecting patient privacy.
• Use clear and organized communication techniques.
• Act in accordance with DaVita's mission and values to deliver high-quality care.

Qualifications:

• High school diploma or GED required.
• At least two years of experience in a professional office environment.
• Ability to handle confidential information with discretion.
• Excellent verbal communication skills over the phone.
• Proficiency with Microsoft Office, Excel, Outlook.
• Commitment to service excellence and patient satisfaction.
• Ability to work independently.

Preferred Qualifications:

• 1-3 years clinical medical office experience.
• CNA or MA experience is a plus.
• Experience coordinating patient care, navigating insurance payors, and working in direct patient care/support.
