Client Care Coordinator

OVERVIEW:
The Client Care Coordinator is responsible for facilitating care coordination services for individuals receiving care from P2’s clients in need of enhanced care coordination services. Ensuring that individuals will receive timely, high-quality, and efficient health care and support services through the development of a multidisciplinary care plan, self-management goals, and referrals to both internal and external resources.
RESPONSIBILITIES:
Below are the tasks of the Care Coordinators:
Individualized Service Plans:
Create and update individualized service plans for all individuals based on state-provided information and any changes in the individual's health. Ensure plans are aligned with state regulations and the individual's evolving needs.Client Binder Audits:
Conduct regular audits of individual’s records to ensure all documents are current and in compliance with state regulations.Address any discrepancies and work towards continuous improvement in documentation processes.Monthly Check Calls:
Conduct monthly check calls to individuals, documenting relevant information and ensuring their well-being.Identify any emerging concerns or changes in the individual’s situation that require attention.Meeting Coordination:
Manage and coordinate quarterly and annual meetings, scheduling sessions with P2 Client’s field supervisors and the operations manager.Facilitate communication and collaboration during these meetings.Client Complaints Management:
Address and manage complaints, documenting issues and coordinating with the operations team to develop action plans for resolution.Ensure timely communication with team members throughout the resolution process.Incident Tracking and Reporting:
Track, investigate, and report all incidents on the state-mandated system (SIMs) within the required time frames.Communicate individual’s new needs to the team promptly and ensure implementation, serving as the primary contact for individuals concerns.Training Coordination:
Coordinate with P2 Client’s training coordinator for any changes in individuals status that may necessitate training or retraining for caregivers.Ensure P2 Client’s caregivers are equipped with the necessary skills to meet the evolving needs of the individuals under the care of P2’s Clients.Referral Management:
Manage referrals to appropriate agencies, when necessary, to support individuals in achieving goals outlined in their care plans.Foster relationships with external service providers to enhance individuals' care.Service Provider Relationships:
Establish and maintain relationships with identified service providers to ensure comprehensive and high-quality care for individuals.Data Collection and Reporting:
Assist in data collection and generate reports as needed for internal and external stakeholders.Collaborate with other team members to enhance the implementation of care plans.Collaboration and Care Management:
Collaborate with other team members in care coordination and management activities, ensuring seamless implementation of care plans.Contribute to continuous improvement in care delivery processes.
WORK SCHEDULE: Monday-Friday 9 AM - 5 PM EST
