Key Responsibilities:
Conduct psychosocial assessments of patients and their families to identify their unique needs.
Assist patients applying for guarantee letters to different government agencies and foundations
Develop individualized care plans and interventions to address emotional, social, and financial challenges.
- Provide counseling and support to patients and families facing illness, trauma, or end-of-life care.
- Collaborate with medical professionals to ensure a comprehensive approach to patient care.
- Advocate for patient rights and assist in navigating healthcare systems and resources.
- Maintain accurate and confidential patient records and documentation.
Job Type: Full-time
