Roles and Responsibilities:

A Healthcare Case Agent coordinates appointments by acting as a central administrative point of contact for patients, scheduling necessary medical appointments with various providers within their healthcare plan, ensuring patients attend scheduled appointments, and generally the overall care coordination process to optimize patient outcomes.

  • Work with US based health case managers ensuring that follow-up visits, necessary tests, and levels of service and referrals are set according to the patient's care plan.
  • Sending timely reminders to patients about upcoming appointments to improve attendance.
  • Ensuring seamless transitions between different healthcare providers and coordinating care across various specialties.
  • In some cases, helping patients arrange transportation to appointments if needed.
  • Providing patients with information about their appointments, community service referrals/resources, and prepare status report.

Qualifications (Experiences and skills):

We are looking for someone who has a genuine desire to help customers with a positive attitude and empathy. A sense of urgency along with strong organizational and multitasking skills will be necessary for this administrative role.

  • Completed Bachelor's Degree in Health Education, Nursing, Social Work, Psychology (or related healthcare degree)
  • Excellent English written and verbal communication abilities
  • Preferred minimum 1 year experience in the healthcare/assisted nursing/social services sector with a combination of relevant coursework, internships, training, and work experience
  • Exceptional follow-up skills and highly organized
