Job Description:

The Medical Records Personnel is responsible for managing and maintaining patient medical records in accordance with hospital policies, regulatory requirements, and confidentiality standards. This position ensures the accuracy, security, and accessibility of medical records, supporting the healthcare team in providing quality care to patients. The Medical Records Personnel will also facilitate the smooth operation of health information processes, ensuring compliance with privacy laws and organizational procedures.

Key Responsibilities:

Record Management:

  • Maintain and update patient medical records, ensuring accuracy, completeness, and compliance with legal, regulatory, and organizational requirements.
  • Process incoming patient records, including patient admissions, transfers, and discharges.
  • Ensure medical records are accurately filed and organized for easy retrieval.
  • Regularly audit records to ensure they meet compliance standards.

Data Entry & Documentation:

  • Input patient data into the hospital's electronic health record (EHR) system or paper records, ensuring accuracy and completeness.
  • Transcribe physician notes, diagnostic reports, and other medical documents into the appropriate patient record.
  • Prepare reports and summaries as required by physicians, staff, and management.

Compliance and Confidentiality:

  • Adhere to healthcare regulations and guidelines, including HIPAA (Health Insurance Portability and Accountability Act), to ensure patient confidentiality and privacy.
  • Ensure that medical records are properly secured and stored in accordance with hospital policies and legal requirements.
  • Respond to requests for information, ensuring that all releases of medical records are handled in compliance with legal and organizational policies.

Collaboration & Communication:

  • Work closely with healthcare providers (doctors, nurses, and specialists) to ensure accurate and complete documentation.
  • Assist in the coordination of patient information for case management, insurance claims, and billing purposes.
  • Collaborate with the IT department to troubleshoot electronic medical record (EMR) system issues and ensure smooth operation.

Training and Support:

  • Provide guidance to staff on proper record-keeping procedures and policies.
  • Participate in training programs to stay up to date with industry standards and hospital protocols.

Other Duties:

  • Perform general clerical duties such as answering phones, responding to inquiries, and scheduling appointments as needed.
  • Assist in preparing medical records for audits and inspections by regulatory bodies.

Qualifications:

  • High school diploma or equivalent required; Associate's or Bachelor's degree in Health Information Management, Medical Records, or related field preferred.
  • Certification as a Registered Health Information Technician (RHIT) or Certified Medical Record Technician (CMRT) is a plus but not required.
  • Proven experience in medical records management, preferably in a hospital or healthcare setting.
  • Strong knowledge of medical terminology, EHR systems, and data entry procedures.
  • Familiarity with healthcare privacy regulations.
  • Excellent organizational skills and attention to detail.
  • Ability to maintain confidentiality and handle sensitive patient information.
  • Strong communication skills, both written and verbal.
  • Ability to work independently and as part of a team.

Physical Requirements:

  • Ability to sit for extended periods of time and use a computer for data entry.
  • Ability to lift and carry light materials (e.g., patient files, office supplies) as needed.
