HOME HEALTH SCHEDULER (WORK FROM HOME)

The Scheduler plays a vital role in coordinating patient visits, maintaining accurate schedules, and ensuring smooth communication between field clinicians, office staff, and referral sources

Qualifications:

  • Must have Scheduling experience in Home Health using Kinnser

Responsibilities:

  • Plot visits in Kinnser
  • Review Frequency and Duration in Kinnser
  • Contact clinicians for their schedule and frequency and duration
  • Check CareStitch notes
  • Coordinate and manage schedules for field staff, ensuring patients are seen promptly and according to their care plan
  • Maintain up-to-date scheduling records, logs, and client rosters
  • Communicate scheduling changes promptly to field and office staff
  • Verify visit frequency and compliance after notes are matched to billing records, reporting discrepancies to the supervisor immediately
  • Prepare and distribute weekly field staff schedules
  • Assist with coordination of patient services and relaying messages among clinicians, office staff, and liaisons
  • Support referral intake processes by preparing documentation and distributing copies to appropriate team members
  • Assist with processing therapy notes, obtaining visit counts, and reporting weekly summaries to the Director of Nursing

Job Type: Full-time

Pay: Php292.74 per hour

Expected hours: 40 per week
