Job Description

As a Care Coordinator, you will ensure seamless service delivery for clients by managing schedules, supporting carers, and providing exceptional communication and care coordination. You'll play a vital role in creating a positive experience for both clients and service providers.

The Qualifications We Seek

• 1 to 3 years of experience
• Experience in a similar role, preferably within the Disability or Aged Care sectors
• Ability to work independently and achieve results in a fast-paced environment
• Strong problem-solving and multitasking skills
• Familiarity with client management systems
• Proficient in Microsoft Word, Excel, SharePoint, and Outlook
• Understanding of the National Disability Insurance Scheme (NDIS)

Your Daily Tasks

• Facilitate the whole service delivery for clients
• Manage the roster of carers and support carer engagement to ensure optimal use of their availability
• Assist carers with accessing and using the electronic rostering system
• Respond to carer enquiries
• Be the first point of contact for client enquiries, incidents, complaints, and feedback
• Confirm and manage bookings with clients and service providers
• Maintain regular contact to ensure client satisfaction and wellbeing
• Provide progress reports to families, carers, and stakeholders
• Monitor client funding and advise on the best use of available resources
• Follow up on client renewals, service holds, and opportunities for re-engagement
• Receive and manage incoming calls and service requests
• Participate in the after-hours roster as required
• Perform other duties as required
