About the role

Talent Outsourcing Channel Solutions' is seeking an experienced Care Coordinator / Nurse to join our team in Davao City, Davao del Sur. This is a full-time position that will play a crucial role in the management and delivery of our community-based services.

As the Care Coordinator / Nurse, you will be responsible for coordinating and overseeing the provision of care to our clients, ensuring they receive high-quality support that meets their individual needs. Your role will involve working closely with our field services team and recruiting and managing a network of qualified professionals to deliver exceptional care.

What you'll be doing

  • Coordinating and managing the care and support services provided to clients in the community
  • Recruiting, training, and supervising a team of care professionals to deliver field-based services
  • Assessing client needs and developing individualised care plans to meet their requirements
  • Maintaining detailed records and documentation to ensure compliance with regulatory standards
  • Collaborating with various stakeholders, including healthcare providers and social services, to ensure seamless care integration
  • Monitoring and evaluating the quality of care delivered to clients, making improvements as needed
  • Providing guidance, support, and mentorship to the field services team
  • Contributing to the overall strategic direction and growth of the community services program

What we're looking for

  • Hold a degree in Nursing, Healthcare Administration, Human Services, or a related field.
  • Minimum of 2 years' experience in a care coordination or case management role, field services, or recruitment.
  • Proven track record of recruiting, training, and managing a team of care professionals
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and external stakeholders
  • Strong problem-solving and decision-making skills, with the ability to think critically and adapt to changing situations
  • Proficient in the use of relevant technology and software for care coordination and record-keeping
  • Passionate about delivering high-quality, person-centred care and supporting vulnerable individuals in the community
