About the Role

We are seeking a skilled Health Services Navigator to join our team at Private Advertiser. In this role, you will serve as a key liaison between our Care Management programs and members, providing critical support and guidance to help them navigate the healthcare system.

Responsibilities
• Develop and maintain relationships with members, hospitals, health plans, physicians, and other stakeholders to ensure seamless care coordination.
• Gather and record member information in accordance with policies and procedures via telephone.
• Provide information about program features and services to encourage member participation in Care Management programs.
• Collaborate with clinical staff to develop and implement effective care plans.
• Initiate referrals and manage consults from clinicians for participants registered in Care Management.
• Gather information from various sources, including hospitals, health plans, physicians, patients, vendors, and other referral sources.
• Contact physician offices to obtain demographic information or related data as needed.
• Mentor less experienced Health Program Advocates.

Requirements
The ideal candidate will have a high school diploma or equivalent and a minimum of 6 months experience; or any combination of education and experience which would provide an equivalent background.
