Nurse Care Specialist

We are actively hiring nurses to join a healthcare initiative focused on care coordination and chronic disease management. As a Care Coordinator, you will play a critical role in improving the health outcomes of patients in the United States — helping them avoid unnecessary hospitalizations through proactive support, education, and follow-up care.
Job DescriptionThis is a full-time night shift role (aligned with US Eastern Time), ideal for nurses who are highly organized, fluent in English, and committed to making a difference in patients' lives.
Serve as the primary care coordinator for assigned US-based patients. Conduct telephonic outreach and/or in-person assessments (based on program guidelines). Collaborate with physicians, specialists, and team members to create individualized care plans.
Required Skills and Qualifications- Active RN license
- Experience in chronic care, patient education, or case management
- Excellent English communication skills
- Strong clinical judgment and attention to detail
- Comfortable working independently and using telehealth systems
