Key Responsibilities:
  1. Quality Improvement: Develop and implement quality improvement initiatives to enhance patient care and services.
  2. Safety and Risk Management: Coordinate and educate staff on safety and risk management protocols to ensure a safe work environment.
  3. Employee Orientation and Competency: Assist in the development and implementation of employee orientation and competency programs to ensure staff are equipped to provide high-quality care.
  4. Regulatory Compliance: Ensure the organization's compliance with regulatory and accreditation standards, including Medicare Conditions of Participation and CHAP standards.
  5. Clinical Resource: Provide clinical guidance and support to staff to ensure high-quality patient care.
  6. Community Education: Collaborate with the community education team to provide education and outreach to the community.
Requirements:
  • Registered nurse with current licensure to practice professional nursing in the State.
  • Bachelor's degree in Nursing from an accredited program.
  • Three to five years of clinical nursing experience, with an emphasis in home care/hospice nursing preferred.
  • Excellent understanding of performance improvement, quality assessment, and utilization management.
  • Self-directed and able to work with minimal supervision.
  • Excellent coordination and communication skills.
