Nurse Care Coordinator

We are actively hiring Registered Nurses to join a major healthcare initiative focused on care coordination and chronic disease management.
As a key member of our team, you will play a critical role in improving health outcomes of patients through proactive support, education, and follow-up care. Your primary responsibility will be to coordinate patient care across various healthcare settings, ensuring seamless transitions and minimizing gaps in treatment.
This is a full-time night shift role ideal for nurses who are highly organized, fluent in English, and committed to making a difference in patients' lives. You will work closely with multidisciplinary teams to develop and implement individualized care plans that address the unique needs of each patient.
- Develop and maintain accurate and comprehensive patient records, including medical histories, treatment plans, and progress notes.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care delivery.
- Collaborate with other healthcare professionals to coordinate patient transfers, appointments, and diagnostic testing.
