Job Title: Patient Care Specialist

Job Summary:

As a Patient Care Specialist, you will provide exceptional patient care and transportation services to our residents. This is a dynamic role that requires strong communication and teamwork skills.

Key Responsibilities:

  • Transport residents from the nursing home for their routine medical appointments and admission/discharge
  • Coordinate with the healthcare team in the planning of the route and time of transport.
  • Maintain the cleanliness and working condition of the van.
  • Attend to any other delivery requirements or errands when needed.

Care Assistant Role:

  • Assist the registered nurses in delivering high-quality care to the residents in the Home.
  • Support the physiotherapist in delivering therapeutic activities to the residents in the Home.
  • Help maintain a clean, safe, and neat environment for the residents in the nursing home.

Requirements:

  • Relevant Patient Care course with certification
  • AED/BCLS Certified
  • Ability to work under high-pressure and fast-paced environments
