Clinical Documentation Specialist

We are seeking a highly skilled medical scribe to join our team. As a medical scribe, you will play a vital role in supporting the healthcare process by accurately documenting patient encounters.
Your primary responsibility will be to capture relevant information during or immediately after visits, ensuring that notes are clear, structured, and ready for provider sign-off.
You will work closely with our team to ensure timely chart updates and maintain consistency of records across encounters.
- Proven experience as a medical scribe, virtual assistant, or healthcare documentation specialist is required.
- A strong written and spoken English communication skillset is essential.
- Typing speed of at least 45 WPM with high accuracy is necessary.
- A working knowledge of basic medical terminology is required.
- Proficiency with Electronic Health Record (EHR) documentation and Microsoft Office (Word, Excel, Outlook) is expected, with strict adherence to HIPAA regulations.

Requirements:
- Accurately document patient encounters into the EHR during or immediately after visits.
- Capture relevant history, objective findings, assessments, treatment plans, and outcomes in clear, structured notes.
- Prepare and edit patient notes, visit summaries, progress reports, and letters to referring physicians.
- Maintain consistency of records across encounters and ensure timely chart updates.
- Organize and track outstanding documentation for provider review.
Work Setup & Hours
- Remote position.
- Must have a reliable internet connection and a quiet workspace.
- Required to provide own computer with Intel Core i5 or equivalent or higher.
- Hours: 40 hours per week, Monday–Friday, 11:00 a.m.–8:00 p.m. Eastern Time.
