Home Health Clinical Care Specialist

The primary objective of the Remote Clinical Liaison position is to provide clinical support to the Director of Nursing in managing home care services. This role entails reviewing patient care plans, ensuring compliance with regulatory standards, and coordinating nursing visits to guarantee efficient program execution.
">Key Responsibilities:- Clinical & Administrative Support:
- Reviews patient care plans, MARs, and physician orders for accuracy and compliance.
- Follows up with physicians for signed orders and ensures proper transcription into patient records.
- Supports the Director in tracking missed or overdue nursing visits and coordinates completion.
- Assists with client admissions and discharges by preparing and verifying required documentation.
- Generates and maintains compliance reports to assist with audit readiness.
- Care Coordination:
- Monitors and manages scheduling workflows to align PDN visits with care plans.
- Serves as a remote liaison between nurses, patients, families, and providers to streamline communication.
- Tracks staffing assignments and availability to support resource allocation.
- Ensures staff credentialing (licenses, CPR, TB, etc.) are tracked and up to date.
- Quality Assurance & Compliance:
- Supports quality improvement initiatives by compiling reports and identifying documentation gaps.
- Assists in reviewing clinical documentation for accuracy, completeness, and compliance with state/federal regulations.
- Provides feedback to field nurses on documentation corrections or updates.
- Training & Education Support:
- Helps facilitate remote orientation and training sessions for nursing staff.
- Tracks completion of required in-services and continuing education.
- Assists in distributing policy updates, clinical reminders, and compliance alerts to nursing staff.
